Please read all the instructions carefully before filling the applications.
Guidelines on the application for a replacement passport
- Applications for new passports in lieu of a lost/ damaged passport should be accompanied with the following documents:
- Form “K” (in duplicate)
- Photo/ Signature Card (IM35 B) with 3 colour photographs of size 35mm x 45mm
- Declaration form (Form D). Please complete in BLOCK LETTERS.
- Application for addition/ deletion of children’s names where necessary. (Form IM 35C)
- Present Sri Lankan passport
- Birth Certificate (original is required)
- National Identity Card (original is required)
- Marriage Certificate where applicable (original is required)
- Written confirmation from the relevant area police to the effect that the loss of passport has been reported (Police Report)
- Applications must be submitted in duplicate and the address in the UAE and telephone number should be clearly stated.
- Please refer to Consular Fees section for relevant fee for obtaining a new passport in lieu of a lost passport.
- Fee shall be paid by cash and please note that cheques will not be accepted.
- All other information should be furnished as required in the application Form “K”.
